Change of Address Request Template
Complete the Change of Address Template when a bank customer gets a new address, and be sure the old address is noted. Obtain the new address and new phone numbers for work and home. Note the effective date of the change. The bank employee processing the change of address request must sign the form.
Note whether the request was in person, over the phone, or by mail. If it was in person, the banking customer should sign the BNK107-2 CHANGE OF ADDRESS FORM. If the request was over the phone or by mail, note this on the customer’s signature line. Attach the mail request to the form.
Ask the customer if this change of address affects any other accounts. The customer requesting the change may make changes to other accounts if and only if the signers on the other accounts have the same old address as the one given and if the social security number for the other customer is given. If the other party has individual accounts, the address cannot be changed except by authorization given by that individual in person, over the phone, or by mail. This information should also be noted on the form.
Check to see if the change affects any other services, such as credit cards, or safe deposit boxes. If customers would like to change their address to a Post Office box, they must also have a physical address. Also check to see if an alternate address exists on the account and whether it should stay the same, be deleted or changed.
