The Supplement to Employment Application Template is needed for statistical purposes to meet Federal Affirmative Action reporting requirements on Equal Employment Opportunity. In order for the company to comply with federal government regulations, it is necessary for the company to compile and maintain detailed information on each formal candidate for employment and those who are hired.
Once this data is collected, then the HRG103-2 EMPLOYMENT APPLICATION SUPPLEMENT should be detached and kept as a separate, confidential record, apart from regular personnel records. A pre-employment inquiry about a disability is allowed if required by another Federal law or regulation such as those applicable to disabled veterans and veterans of the Vietnam era. Pre-employment inquiries about disabilities may be necessary under such laws to identify applicants or clients with disabilities in order to provide them with required special services.
At the end of each year, the Human Resources Manager should compile and tabulate the data collected to determine the diversity of the applicant pool versus those hired.
