The company should respond within one or two business days to all requests for Family or Medical Leave using the Request for Leave Employer Authorization Template. CMP103-2 EMPLOYER RESPONSE TO REQUEST FOR LEAVE informs the employee if they are eligible, if it is required for them to furnish medical certification, if they are required to substitute accrued paid leave for unpaid FMLA leave, and more.
Keep in mind unpaid leave will be granted for any of the following reasons:
To care for the employee’s child after birth, or placement for adoption or foster care.
To care for the employee’s spouse, son or daughter, or parent, who has a serious health condition.
For a serious health condition that makes the employee unable to perform the employees job.
