Non-Standard Software Request Template

In some cases, the need for a software solution outside the scope of the Common Operating Environment (COE) will arise. When a system user identifies such a requirement, that user should submit a formal request for permission to install and use the non-standard software program, using ITAM106-1 NON-STANDARD SOFTWARE REQUEST. The Non-Standard Software Request Template should be forwarded via the user’s department director to the Information Systems Manager.

The Information Systems Manager should evaluate and approve requests for non-standard software.

  • Prior to approving the software for use, the Information Systems Manager should ensure that the application is tested for compatibility and interoperability with the company COE.
  • The IS Manager should notify the affected Department manager of acceptance or rejection of the request, giving reasons for rejection.
  • The IS Manager should maintain a file of all ITAM106-1 forms, accepted, and rejected requests, for periodic review.

All personal software is considered unauthorized and cannot be installed or used on any company equipment unless the user submits a Non-Standard Software Request for that software and the request is approved.
